Our office hours are Monday through Thursday from 9am to 5pm. We are closed for lunch between 12:30-1:30pm daily.

We do offer evening appointments every Tuesday until 7pm.

Before Coming to the Center

Please fill out the New Patient Form and bring it with you to the first visit. If you do not have access to a computer/printer, let us know and we will mail the form to you.

Initial visits are generally 90 minutes. A detailed plan of action will be developed at the first visit after a comprehensive medical history is taken. Follow up visits range from 30-45 minutes and cost is less. Often there is an initial need for follow up visits every 6-12 weeks or when test results become available and if treatment changes need to be made. Phone consults are available for test results or urgent concerns.

We are not Medicare Providers and accept NO insurances. This practice operates as a Fee for Service Medical Practice. We DO provide Standard Insurance Reimbursement Forms that enable many of our patients to receive 70%-80% out of network reimbursement directly from their insurance company for the cost of the visits and testing.

The Center for Preventive Medicine’s success, has been due to the patients we have seen and helped. Most of our patients have been referred by other patients and local practitioners.
